1. The vote

   Covered

   Coverage starts January 1, 2027, for non-Medicare members, at a flat $200 a prescription.

2. Where the $200 came from

   Context

   Not a round number somebody liked. It's the ceiling the drugmakers' rebate contracts allow before the rebates start disappearing.

   “New Navitus agreements also allow the Board to establish a dedicated formulary tier for AOMs. Under this option, the Board could require a member copay of up to $200 per prescription without risking rebate loss.”

3. The same contracts set the BMI rule

   Conditions apply

   Tighten who qualifies and the rebates shrink. Tighten past a BMI of 35 and they stop. That's a manufacturer term shaping a public benefit.

   “Under current manufacturer agreements, rebates are reduced if the minimum BMI requirement is increased to 35 and end if the BMI requirement is more than 35.”

4. What it costs the fund

   Context

   The board's actuary priced six years of it before the vote, and the number is not small.

   “Segal estimates that covering AOMs with full rebates would cost the Board between $3.5 million and $24.3 million annually over the first six years of coverage (Table 1).”

5. The cheap headline prices are not for you

   This is the line worth carrying out of the room. The direct-to-consumer deals announced in November 2025 do nothing for someone buying through an insurance plan.

   “It is important to note that these reduced prices apply only to direct-to-consumer purchases. The agreement does not affect pricing for individuals using non-Medicare insurance such as that offered by the Board.”
