SoonerCare's three doors to a GLP-1
Oklahoma's Medicaid program files the anti-obesity GLP-1s under Metabolic Disorders rather than diabetes, and writes three separate criteria blocks for them. Every one of the three contains a sentence that rules out the obvious reason a person would ask. Getting through any of them also means a fresh prior authorization at every dose along the way.

The diabetes ladder starts with metformin
Conditions applyOn the diabetes side the GLP-1s sit in a tier table, and no GLP-1 is in the first tier. Reaching the second one means three months of metformin first.

What the state didn't publish
No page in SoonerCare's published 2026 criteria approves a GLP-1 for obesity on its own. The two anti-obesity products appear on exactly one of the twenty-one category pages, and every block that names them rules out obesity without a second diagnosis.
We looked at 21 documents to say that — all 21 therapeutic-category criteria pages linked from SoonerCare's 2026 prior-authorization index, each fetched and read as text in a single pass.
The identical scan, in the same run, found Wegovy 30 times and Zepbound 18 times on the Metabolic Disorders page and zero times on the other twenty, Victoza 10 times and Mounjaro and Ozempic twice each on Diabetes/Endocrine, and the phrase Approval Criteria 2,573 times across the set — the ruler reads these pages. Against those same 21 pages, Saxenda and the phrase anti-obesity each returned zero. ⚠ This census overturned an earlier reading of this state: the diabetes page alone would have supported a false claim that SoonerCare names no anti-obesity GLP-1 at all.
