Wegovy cost without insurance: pen, pill and HD
Without insurance, the Wegovy pen costs $349 a month at any dose up to 2.4 mg, after two introductory months at $199 for new patients. The higher-dose Wegovy HD pen, 7.2 mg, costs $399. The Wegovy pill costs $149 a month at 1.5 mg, $199 at 4 mg and $299 at 9 mg and 25 mg. Those are the self-pay prices at NovoCare Pharmacy, Novo Nordisk's own pharmacy, and TrumpRx lists the same figures.
What this comes down to
- Pen: $199 for each of the first two monthly fills at 0.25 mg and 0.5 mg (new patients, through December 31, 2026), then $349 a month. Wegovy HD 7.2 mg: $399 a month.
- Pill: $149 at 1.5 mg, $199 at 4 mg and $299 at 9 mg and 25 mg. At the top doses the pill is $50 a month cheaper than the pen.
- Wegovy HD was approved on March 19, 2026, for people who have been on 2.4 mg for at least 4 weeks and need more weight loss. In its trial it averaged 18.8% weight loss over 72 weeks against 15.5% on 2.4 mg.
- Some telehealth companies charge far more for the same pen: six that we review list Wegovy at $1,349 to $1,839 a month. Ask where the prescription will be filled before you pay.
- Pill, 1.5 mg$149
- Pill, 4 mg$199
- Pill, 9 mg or 25 mg$299
- Pen, first two fills$199
- Pen, up to 2.4 mg$349
- Wegovy HD pen, 7.2 mg$399
A month of the pen is a box of four pens; a month of the pill is a bottle of 30 tablets.
How much is Wegovy a month without insurance?
The lowest cash price for brand Wegovy is Novo Nordisk's own, at NovoCare Pharmacy. A month of the pen is one box of four weekly pens; a month of the pill is one bottle of 30 tablets. Every dose needs a prescription, and NovoCare can connect you with a prescriber if you do not have one. Our NovoCare Pharmacy review has every price on the page.
| Form and dose | A month |
|---|---|
| Pen, 0.25 mg or 0.5 mg, first two fills for new patients | $199 |
| Pen, 0.25 mg to 2.4 mg, after that | $349 |
| Wegovy HD pen, 7.2 mg | $399 |
| Pill, 1.5 mg | $149 |
| Pill, 4 mg | $199 |
| Pill, 9 mg or 25 mg | $299 |
Wegovy self-pay prices at NovoCare Pharmacy, October 2026
The $199 pen price covers two monthly fills for people new to the offer and runs through December 31, 2026. After that, every pen dose from the 0.25 mg start to the 2.4 mg maintenance dose costs the same $349, so the price does not climb as the dose steps up the way it does for some other GLP-1 drugs. What the dose steps cost at every seller shows where that is not the case. If you are deciding between the two forms, the Wegovy pill versus the injection compares the daily routine, the trial results and the side effects.
Is Wegovy cheaper on TrumpRx?
No, it is the same price. When TrumpRx launched in February 2026, the White House said injectable Wegovy would be available there for as low as $199 a month and the pill for as low as $149, depending on dose[7]. On October 5, 2026, TrumpRx listed the pen at $199 for the first two fills, then $349, with Wegovy HD at $399[8], and the pill at $149, $199 and $299 by dose[9]. Those match NovoCare's prices exactly. The difference is how you use them: TrumpRx gives you a coupon to show at a retail pharmacy, while NovoCare ships the drug to you. What TrumpRx lists for every GLP-1 covers the rest of its list, including Ozempic and Zepbound.
What is Wegovy HD 7.2 mg, and what does it cost?
Wegovy HD is a higher weekly dose of the same drug, 7.2 mg of semaglutide in one pen. The FDA approved it on March 19, 2026, as a supplement to the original Wegovy approval[2]. The label allows it only for adults who have taken 2.4 mg for at least 4 weeks and need more weight loss[4]. Self-pay, it costs $399 a month at NovoCare Pharmacy, $50 more than the standard pen.
In the 72-week trial behind it, 1,407 adults with obesity and without diabetes were randomized to 7.2 mg, 2.4 mg or placebo. Average weight loss was 18.8% on 7.2 mg, 15.5% on 2.4 mg and 3.9% on placebo[4]. That trial is STEP UP, published in The Lancet Diabetes & Endocrinology[5]. The extra weight loss comes with more side effects. The one most specific to the high dose is dysesthesia, an altered or burning sensation of the skin:
| Side effect | Placebo | 2.4 mg | 7.2 mg |
|---|---|---|---|
| Nausea | 13% | 35% | 39% |
| Vomiting | 6% | 16% | 22% |
| Dysesthesia (altered skin sensation) | 0% | 6% | 22% |
| Constipation | 8% | 19% | 20% |
| Hair loss | 1% | 3% | 6% |

What do telehealth companies charge for brand Wegovy?
Three of the companies we review list the Wegovy pen at or under NovoCare's $349: ZipHealth, Knownwell, and Hers. Some brand sellers add a fee on top: Hims requires its membership, $149 a month after the first, and ZipHealth adds a $79 service fee per treatment. Six others list Wegovy at $1,349 to $1,839 a month: Trimrose, RxSpan MD, altRX, Lemonaid Health, Levity, and OrderlyMeds. That is the same drug at several times the drugmaker's own cash price, so if a brand prescription is what you want, you can ask for it to be sent to NovoCare Pharmacy or to a retail pharmacy that takes the coupon.
Is compounded semaglutide cheaper than Wegovy?
Usually. At the telehealth companies below, compounded semaglutide month to month runs from $59 to $299 a month for the medication, against $349 for the Wegovy pen. It is not the same product: compounded semaglutide is not FDA-approved, it is made by a compounding pharmacy rather than Novo Nordisk, and its quality depends on that pharmacy. Where compounded semaglutide stands with the FDA explains the rules, and the price gap, drug by drug shows what you give up for the savings.
| Company | Medication a month | Membership or fee |
|---|---|---|
| Collective | $59 | $199 a year |

Read the review before you sign up, because plan length, first-month promotions and what month two costs differ a lot: Collective, CoreAge Rx, Eden, SnagRx, YourEra, Sprout Health, bmiMD, and Trimi. The compounded semaglutide price board lists every company we review, with first-month and ongoing prices side by side.
Can insurance or Medicaid pay for Wegovy instead?
Sometimes, and the label is the reason to ask. Wegovy injection is approved not only for weight loss but also to lower the risk of heart attack, stroke and cardiovascular death in adults with established heart disease and overweight or obesity, and for a form of fatty liver disease with liver scarring[4]. The heart indication rests on SELECT, a trial of 17,604 adults[6]. A plan that excludes weight-loss drugs may still cover a drug prescribed for one of those other uses. What the heart indication means for coverage goes through it, prior authorization, step by step covers what a plan usually asks for, and Medicaid GLP-1 coverage by state shows where Medicaid pays. The pill was approved separately, on December 22, 2025[3]; the original Wegovy pen dates to June 4, 2021[1].
