Wegovy pill vs injection: weight loss, side effects and monthly cost

The Wegovy pill and the Wegovy injection are the same drug, semaglutide, and in their own trials they produced similar weight loss: 13.6% over 64 weeks on the 25 mg daily pill and 14.9% over 68 weeks on the 2.4 mg weekly shot, in separate trials. The pill is taken every morning on an empty stomach; the shot is once a week. Self-pay from NovoCare Pharmacy, the pill costs $149 to $299 a month by dose, and the pen costs $349 a month after two introductory months at $199.

What this comes down to

  • Weight loss is close: 13.6% at 64 weeks on the pill (placebo 2.4%) and 14.9% at 68 weeks on the shot (placebo 2.4%). No trial has compared the two forms directly.
  • The pill comes with a daily routine: an empty stomach, no more than 4 ounces of plain water, and a 30-minute wait before food, drinks or other pills. The shot can be taken any time of day, with or without meals.
  • At the maintenance dose, the 25 mg pill is $299 a month self-pay and the 2.4 mg pen $349. TrumpRx lists the same prices.
  • You can switch either way. The label says to start the 25 mg pill one week after the last 2.4 mg shot, or the 2.4 mg shot the day after the last 25 mg pill.

Does the Wegovy pill work as well as the injection?

On the numbers each form's own trial produced, roughly yes. The pill was approved on December 22, 2025[1], four and a half years after the injection[2]. In the pill's 64-week trial, adults taking 25 mg a day lost an average of 13.6% of their body weight against 2.4% on placebo, and 59.8% lost at least a tenth of it. In the injection's 68-week trial of 1,961 adults, 2.4 mg a week produced 14.9% against 2.4%, and 66.1% lost at least a tenth[3].

Those trials are OASIS 4 for the pill[4] and STEP 1 for the injection[5], both in the New England Journal of Medicine. They enrolled different people over different lengths of time, so the small gap between them is not a measured difference. What changes in practice is the routine and the price, not the drug.

Taking each form, from the Wegovy label

The pill's routine is not a suggestion. Semaglutide is a peptide that the stomach breaks down, and the tablet's absorption depends on taking it exactly this way[3]. If a daily, food-free morning is hard to keep, that matters more than any difference in the trial numbers. Foundayo, the other approved GLP-1 pill, can be taken with or without food.

Does the pill cause fewer side effects than the injection?

Not according to the label. It says the types and frequency of common side effects in the pill's trial were similar to those seen with the injection: mostly nausea, diarrhea, vomiting and constipation. In the pill's trial, 6.9% stopped because of side effects against 5.9% on placebo; across the injection's main trials it was 6.8% against 3.2%[3]. The OASIS 4 paper counted any stomach-related event in 74.0% of people on the pill and 42.2% on placebo[4].

At maintenance, the 25 mg pill is $299 and the 2.4 mg pen $349, a difference of $50 a month. The pen's $199 offer covers two monthly fills for new patients through December 31, 2026. TrumpRx, the federal drug-price site that launched in February 2026, lists the same figures: the pill at $149, $199 and $299 by dose[8], and the pen at $199 for the first fills, then $349, with Wegovy HD at $399[9], matching the White House's "as low as" $149 for the pill and $199 for the shot[7]. Compounded semaglutide tablets are not the Wegovy pill

Some telehealth companies sell compounded semaglutide tablets. They are not FDA-approved, they were not tested in OASIS 4, and they may not contain the absorption enhancer the approved tablet relies on. Can you switch from the Wegovy injection to the pill?

Yes, and the label says how. From the 2.4 mg weekly shot, start the 25 mg pill one week after your last injection. From the 25 mg pill, start the 2.4 mg shot the day after your last tablet; if 25 mg tablets are not tolerated, the label suggests the 1.7 mg shot instead[3]. The diabetes tablets RYBELSUS and the Ozempic pill are also oral semaglutide, but they are approved for type 2 diabetes, not weight loss, and come in different strengths[6]. Switching GLP-1 providers covers moving between sellers rather than forms.

Is the Wegovy pill as effective as the Wegovy shot?

In their own trials the results were close: 13.6% average weight loss over 64 weeks on the 25 mg pill and 14.9% over 68 weeks on the 2.4 mg shot. They were separate trials, so this is not a direct comparison.

Do you have to take the Wegovy pill on an empty stomach?

Yes. Take it in the morning on an empty stomach with up to 4 ounces of plain water, swallow it whole, and wait at least 30 minutes before eating, drinking anything else or taking other oral medicines.

Is the Wegovy pill cheaper than the injection?

Can I switch from Wegovy injections to the pill?

Is Rybelsus the same as the Wegovy pill?

Both are oral semaglutide made by Novo Nordisk, but RYBELSUS and the Ozempic pill are approved for type 2 diabetes in lower strengths, while the Wegovy pill is approved for weight loss at doses up to 25 mg.
