What it is
A membership that sells two compounded GLP-1s and nothing else — semaglutide and tirzepatide, weekly injections, filled by pharmacies it calls regulated and third-party tested but never names. It is run by Health For Everyone Inc., trading as CollectiveOS, out of Houston, and founded by the person who started Thrive Market, who signs a letter on the homepage explaining the pricing as a buying-power argument. The medication is compounded rather than brand-name, which the site says plainly.
The catalog really is two products. The sitemap holds three URLs, the homepage links fourteen, and one of those three sitemap pages serves text identical to the homepage. The Katalys program this runs under is registered as "Collective Peptides"; there are no peptides on it. Microdosing gets one question on the homepage, pointing at the same two drugs at the same two prices, with no separate rate of its own.

What it actually costs
$59 a month for semaglutide, $69 for tirzepatide, identical at every dose — plus $199 a year for the membership, after 21 free days.
There is no plan ladder here. No six-month rate, no annual prepay, no first-month teaser, no surcharge as the dose climbs — the one figure is the figure, and it survived a walk through the signup to the checkout summary, which prints the same number beside the exact date the first membership charge lands. That is rarer than it sounds; most of this market's headline price is a prepay rate wearing a monthly label.
The membership is the part worth doing arithmetic on. It is charged once a year rather than monthly, which is the honest version of a program fee, and the site discloses it in the same sentence as the drug price almost everywhere it appears. Almost. The browser tab reads "Doctor-Guided GLP-1 Weight Loss at $59", and the hero underneath it says "Get GLP-1s at $59/month" with the membership nowhere in frame.
Then there is a charge with no number on it. Section 5 of the terms says CollectiveOS "may charge a nominal, uniform technology transaction fee for each electronic prescription transmission, regardless of the pharmacy selected." We read the terms, the privacy policy, the homepage and every intake screen through checkout and found no amount for it anywhere. The homepage promises "no hidden costs" and "no surprise increases" on the same site.
Who it's wrong for
- Anyone who wants one month and then out. The medication has no commitment, but the membership is an annual charge, and canceling in month four does not buy back nine months of it.
- Anyone who needs to know which pharmacy fills the prescription. Collective names none, on any page we read, and names no prescribing clinician either.
- Anyone who needs written confirmation their state is served. The signup and the checkout both let you pick any state and the District of Columbia, but a menu is a form, not a license — and no page here states where the company operates.
- Anyone who wants a brand-name GLP-1. Both products are compounded; there is no Wegovy or Zepbound route on this site.

There is no plan ladder, no prepay discount and no dose-based escalation on this site at all, which is the trick most of this market runs. The $199 annual membership is disclosed in the same sentence as the drug price rather than discovered later, and the checkout prints the date the first membership charge lands. What holds it at B is a gap between the headline and the contract.
