Zepbound self-pay prices at LillyDirect, vial or KwikPen, October 2026

The lower price on the maintenance doses comes from what Lilly calls its Zepbound Journey Program, and it holds only while each prescription is refilled within 45 days. A gap, a missed shipment or a pause for side effects can turn a $449 month into a $699 one. Vial, KwikPen or autoinjector: which can you buy without insurance?

Zepbound's label lists four forms: a single-dose pen, a single-dose vial, a multi-dose vial and the KwikPen, each in 2.5 mg to 15 mg strengths[2]. For self-pay buyers at LillyDirect it comes down to two:

  • Single-dose vial: one dose per vial, four a month, drawn up with a new needle and syringe each week. Sold only through LillyDirect self-pay, delivered or picked up at a Walmart pharmacy.
  • KwikPen: one multi-dose pen that holds the whole month's four doses, with a new pen needle each week. Same price as the vial, delivered or picked up at a Walmart pharmacy.
  • Single-dose autoinjector pen: the familiar pen with a hidden needle. LillyDirect offers it for people whose commercial insurance covers Zepbound, with a savings card that can bring the cost to as little as $25 a month for up to a three-month prescription. Government plans are excluded.

Is Zepbound cheaper on TrumpRx?

No, it is the same price. TrumpRx launched in February 2026, and the White House said Zepbound would be available there for as low as $299 a month depending on dose[4]. On October 5, 2026, TrumpRx listed only the Zepbound vial, at $299 for 2.5 mg, $399 for 5 mg and $449 for 7.5 mg to 15 mg, labeled as an out-of-pocket price[5]. Those are LillyDirect's own vial prices, so the choice between the two is about how you want the order handled, not the cost.

Why does the price go up as the dose goes up?

Because the label builds the dose up in steps. Everyone starts at 2.5 mg once a week for 4 weeks, moves to 5 mg, and can then go up by 2.5 mg at a time, at least 4 weeks apart. The 2.5 mg dose is only for starting; the maintenance doses are 5 mg, 10 mg or 15 mg[2]. In SURMOUNT-1, the 72-week trial behind the approval, average weight loss was 15.0% at 5 mg, 19.5% at 10 mg and 20.9% at 15 mg, against 3.1% on placebo[3]. So most people paying cash spend their first month at $299, their second at $399, and every month after that at $449 if they move past 5 mg. Is compounded tirzepatide cheaper than Zepbound?

Usually. At the telehealth companies below, compounded tirzepatide month to month runs from $69 to $399 a month for the medication, against $299 to $449 for brand Zepbound. It is not the same product: compounded tirzepatide is not FDA-approved, it is made by a compounding pharmacy rather than Lilly, and its quality depends on that pharmacy.
