What it is
A two-state physician practice rather than a national platform: one named doctor, board-certified in obesity medicine, with her NPI printed in the footer of every page. That is a different shape from the anonymous prescriber pool most of this market runs.
Everything sold is compounded semaglutide or compounded tirzepatide. Brand-name pens are reachable only through the third plan, where you buy the drug yourself, so the compounded-versus-brand choice changes which membership you need.

What it actually costs at TOM Weight Loss
Basic is $199 a month and includes compounded semaglutide at any dose up to 2.4 mg a week. Premium is $249 and includes compounded tirzepatide up to 15 mg. Both cards say billed monthly, and the medication pages repeat the figures.
A membership that includes the drug is rare enough to be worth naming. Most sellers here charge a fee and leave the pharmacy bill to you — which is the failure mode a membership fee usually hides.
The third plan is another matter. The a la carte card says $150 a month in its headline and Billed $100 monthly four lines underneath. Two paragraphs later the page says $100 a month, and a footnote further down says $150 a month. Nothing reconciles them.
That plan's per-fill table is genuinely useful: five compounding pharmacies as columns, a four-week-supply price at every dose, semaglutide from $67 to $259 and tirzepatide from $118 to $549. Note the range — the same 10 mg tirzepatide fill is $199 at one pharmacy and $549 at another, which is the spread a headline price never shows you.
Who it's wrong for
- Skip it entirely unless you are in Texas or Utah. Those are the only two states, stated as a restriction rather than a licensing hedge.
- Do not budget from the a la carte headline. The same page gives that plan four prices, and you would want it confirmed in writing before you enroll.
