Semaglutide with B12: the claims, and what was measured
Two claims travel with a B12-compounded GLP-1: it settles the nausea, and it helps with the tiredness. Neither has been tested in this combination — not once. The nearest thing to a test of the second claim is a crossover trial in which twenty singers got a 1,000 microgram B12 injection or salt water and could not tell the difference. The nearest thing to a test of the first is nothing at all.
What this comes down to
- No randomized trial of semaglutide or tirzepatide with B12 exists. The identical filter finds 23 trials of metformin with B12, so the filter can find drug-plus-vitamin trials.
- Eleven records mention a GLP-1 and a B12 term at all. Three of those mention nausea, and none of the three tests the combination.
- A randomized, double-blind, placebo-controlled crossover trial of a 1,000 microgram B12 injection in twenty singers with mild vocal fatigue found improvements after B12 and after placebo that were comparable to each other.
- Promotion by or on behalf of a compounder has to be truthful and non-misleading, and the FDA says it monitors it.
The nausea claim
Nausea in the first weeks is the reason usually given for the vitamin, and it is a claim about an anti-emetic effect. Anti-emetic effects are easy to test — the same filter that finds nothing for a GLP-1 with B12 finds 863 randomized trials of ondansetron and nausea. So the instrument works and nobody has pointed it at this. Whether the combination has been studied at all is the question the companion article answers; what we would add is that the three records mentioning a GLP-1, a B12 term and nausea together are a case report of neuropathy after semaglutide-associated gastrointestinal intolerance, a 2026 review of micronutrient risk on incretin drugs, and a pharmacology review about achieving glycemic control without nausea. None of them tests adding B12 to reduce nausea.

The control is deliberately another drug studied alongside B12, because that is the exact shape of trial being looked for. Metformin has 23, so trials of a drug with B12 are findable and indexed. Unfiltered, a GLP-1 with a B12 term returns 11 records in total; a fabricated drug name returned 0 through the same filter in the same session.
The energy claim, and the one trial shaped like it
B12 injections for tiredness are older than GLP-1s and have been tested on their own terms. The cleanest example is a randomized, double-blind, placebo-controlled crossover trial at an academic voice center: twenty adult singers without hoarseness but with mild vocal symptoms — reduced stamina, vocal fatigue, effort — each received an intramuscular injection of 1,000 micrograms of cyanocobalamin and, after a washout of at least four weeks, an injection of 0.9% saline[1]. Anyone with known or suspected B12 deficiency was excluded, which is the design choice that makes it relevant here.
At the primary time point, 72 hours, the improvements after B12 and after placebo were comparable to each other. The median change on the primary measure was 1 point after B12 and 3 after placebo. People felt better after both.
An injection for tiredness in people who are not deficient is one of the most-sold products in this market. The one trial designed to isolate it found that saline did the same job. An older crossover trial of injectable cyanocobalamin with folic acid and liver extract in chronic fatigue syndrome reached the same place: patients responded to both, with no significant difference between them[2].

We read all nine. Most are not about B12 for tiredness at all — they are trials of carnitine, duloxetine and pemetrexed in which a B12 term and a fatigue term both appear. Three genuinely test a B12 preparation against placebo on a tiredness-type outcome: the singers' trial, the 1989 chronic fatigue crossover, and a 1973 pilot. A count read without its query attached would have overstated this by three times.
What the vitamin is approved to do
Cyanocobalamin injection is an old, approved product with a narrow indication: vitamin B12 deficiency caused by malabsorption. Nothing in that approval concerns nausea from another drug, and nothing in it concerns energy in people whose levels are normal. What a standalone B12 or lipotropic shot is is covered separately, and the answer does not change when the vitamin rides along with a GLP-1.
There is a real question underneath all of this, which is whether GLP-1 drugs cause B12 deficiency the way metformin can. That is exactly why the metformin control is the right one: the metformin-and-B12 literature exists because a genuine drug-induced deficiency was found and then studied 23 times. The equivalent literature for GLP-1s is a 2026 review describing micronutrient risk as a mechanism to monitor, not a measured deficiency with a dose response[3]. A vitamin added preemptively to every vial is not a response to that.
What a seller is allowed to say
Compounded drugs are not approved, are not reviewed for safety, effectiveness or quality before they are sold, and a compounded drug is not a generic[4]. Prescription drug promotion by or on behalf of a compounder still has to be truthful, non-misleading and accurate, and the agency says it monitors promotional material. Two claims to weigh against that standard when you see them:
- "B12 reduces the nausea." No trial of the combination exists, and the three records that mention both and nausea test something else entirely.
- "The B12 gives you energy." Tested in the shape most likely to succeed — a real injection, in people with symptoms, against saline — and it came out comparable to placebo.
- "It is the same as the brand, with a bonus." The FDA lists a company claiming a compounded drug is the same as an approved one among its telehealth red flags, and the added ingredient is also what the copy rule turns on — the companion article works through that.

Does B12 stop GLP-1 nausea?
No randomized trial has tested that. PubMed returns eleven records mentioning a GLP-1 and a B12 term together, three of which mention nausea, and none of the three tests the combination. The identical filter finds 23 trials of metformin with B12, so the search can find trials of this shape.
Do B12 injections give you more energy?
In people who are not deficient, the best-designed trial says no. Twenty singers with mild vocal fatigue received 1,000 micrograms of cyanocobalamin and, separately, saline; improvements after each were comparable. A 1989 crossover trial in chronic fatigue syndrome found the same thing.
Do GLP-1 drugs cause B12 deficiency?
It has not been established the way it has for metformin, where 23 randomized trials exist. The current literature describes micronutrient risk on incretin drugs as a mechanism worth monitoring rather than a measured deficiency, which is a different claim.
Is there an approved semaglutide and B12 product?
No. There is no FDA-approved product combining a GLP-1 with vitamin B12. Anything sold that way is a compounded preparation, made outside the approval process and not reviewed for safety, effectiveness or quality before it is marketed.
